§530.120. Cards

  • a)         A card containing, at a minimum, the following information, will be issued to each beneficiary:

     

    1)         name of beneficiary;

     

    2)         identification number;

     

    3)         effective date of coverage; and

     

    4)         expiration date of coverage.

     

    b)         A card will not be valid for more than one year from the effective date of coverage; however, beginning January 1, 2002, coverage may be extended or restricted for a longer or shorter period of time in order to enable the Department to convert coverage for a beneficiary to a fiscal year basis and have an opportunity to check for erroneous or fraudulent claims.
